BrightStar Care of Concord, Lexington and Woburn, is a leading provider of in-home and facility-based skilled and non-skilled healthcare services. We’re a team of dedicated professionals providing A Higher Standard of Care. We are looking for a compassionate and motivated Occupational Therapist (OT) to join our home health care team. You’ll work one-on-one with clients in their homes, helping them regain independence and achieve their highest level of functional ability through personalized treatment plans.

We serve clients of all ages across Middlesex County, including the towns of Concord, Lexington, Woburn, Bedford, Burlington, and nearby communities.

This role is a Flexible, Work from Home and Per-Diem opportunity and requires travel to client's homes

Responsibilities:

  • Evaluate patient conditions and develop customized treatment plans

  • Restore physical function and support patients with daily living and adaptive skills

  • Deliver occupational therapy treatments in clients’ homes

  • Coordinate with families, caregivers, and the clinical care team to support recovery goals

  • Accurately document care in our electronic medical record (EMR) system

  • Educate patients and families on exercises and safety practices

  • Ensure care delivery complies with BrightStar Care policies, HIPAA, and state/federal regulations

Qualifications:

  • Graduate of an accredited Occupational Therapy program

  • Current Massachusetts OT license in good standing

  • Minimum 1 year of experience as an Occupational Therapist (home health experience preferred)

  • Current CPR certification

  • Valid driver’s license, auto insurance, and reliable transportation

  • Ability to work independently and travel within assigned service area
